Check the engine oil for metal shavings, which indicate bearing material shedding, or a milky, creamy appearance indicating water intrusion. Perform a cylinder balance test by momentarily disabling each spark plug wire individually. If the noise significantly decreases when a specific cylinder is disabled, confirm internal failure such as a rod bearing issue or piston slap in that specific cylinder.
