Bicycle Chain Slippage Due to Bent Derailleur Hanger

This is diagnosed by visually inspecting the rear derailleur from behind the bike. The jockey wheels must be perfectly vertical and parallel to the cassette cogs. Any visible angle or misalignment of the hanger relative to the cassette indicates a bent component that prevents smooth chain tracking.
